SAFE provides a wide range of resources to support our donors and partners, and their philanthropic efforts.  SAFE’s Donor Education Series (DES) is a series of events from the Planned and Creative Giving Offices specifically designed to help philanthropically-minded individuals make educated, informed and healthy decisions about their philanthropy and legacy planning.

Whether you give to SAFE or another charitable cause, our events are always free, open to the public and will always provide attendees with a take-away that will help them become incredible philanthropists in our community.

  • Death Wishes Planning Questionnaire

Create your own estate inventory and make your funeral wishes known.  Adapt the template we’ve named – ‘My Death Wish List’ to your specific needs.  This document gives you the opportunity to list your responsibilities, in order to communicate your needs to those who are most likely to take care of the arrangements in the event of your death, (e.g. the funeral, children, pets, bills, etc).  It will help others handle your affairs in the way that you would like things to be done.

We recommend filling in a ‘My Death Wish List’ prior to drawing up your will, as it could help you to remember all the details you would like to name in your last will and testament.

Please note: This document is not a will.  It is full of all the useful details that are usually not included in a will.

Use the links below to open the ‘My Death Wish List’ template.  We recommend using the excel/word versions, so that you can change it to meet your specific requirements and update it easily any time in the future.  Alternatively, you can print the PDF version and fill it in by hand.
